Lakefield Care deliver person-centred care to support and empower our participants to live their best life. We provide individual supports that are tailored to meet the unique needs of our participants. We have a team of registered Social Workers who can provide Psychotherapy and other Social Work Services. All participants have a dedicated Case Manager to coordinate their needs. Lakefield Care is all-embracing of different cultures and is LGBTIQA+ friendly.
